Credentialing requirements
- Active state medical, behavioral health, or facility license in good standing
- Accepts assigned SharkCare patients on a sliding or fully-funded basis
- NPI on file and ability to submit standard HCFA-1500 / UB-04 claims
- Signed Business Associate Agreement (HIPAA) and data-use addendum
- Quarterly outcomes reporting via the SharkCare provider portal
